pushes upon its nearest neighbor, causing it to be displaced from its
rest position. The displacement of several nearby particles produces
a region of space in which several particles are compressed together. As the particles are pulled away from each
other, a region is created in which the particles are spread apart.Such a region is known as a rarefaction or low pressure region. When a pressure wave reaches the ear, a series of high and low
pressure regions impinge upon the eardrum. The continuous arrival of high and low pressure regions sets the
eardrum into vibrational motion. This is depicted in the animation
below.Since the eardrum is set into vibration by the incoming pressure
wave, the vibrations occur at the same frequency as the pressure
wave.If the incoming compressions and rarefactions arrive more
frequently, then the eardrum vibrates more frequently. When these high amplitude vibrations
impinge upon the eardrum, they produce a very forceful displacement
of the eardrum from its rest position. This high intensity sound wave
causes a large vibration of the eardrum and subsequently a large and
forceful vibration of the bones of the middle ear.Perforated Eardrum Overview
Your eardrum (tympanic membrane) is a thin, oval layer deep in your ear canal. It is called an eardrum because it looks and acts like a drum. The eardrum receives vibrations from the outer ear and transmits them to the small hearing bones, or ossicles, of the middle ear.Because it is so thin, your eardrum can be ruptured or punctured. The hole exposes the middle and inner ear to damage or infection. The tympanic membrane (eardrum) separates the outer ear from the middle ear.The membrane vibrates when sound waves strike it, and this starts the process that converts the sound wave into a nerve impulse that travels to the brain. When the eardrum is damaged, the hearing process is interrupted. The eardrum also acts as a barrier to keep outside material (such as bacteria) from entering the middle ear.Foreign objects in the ear are another cause of perforated eardrum.Ear infections may cause a ruptured eardrum as the pressure of fluid in the middle ear increases.Conversely, a ruptured eardrum can cause ear infections because the eardrum is no longer intact, and bacteria can enter the middle ear.If the eardrum is punctured, the doctor will see an opening in it, and may even see the bones of the middle ear.Sometimes it is hard for the doctor to see the eardrum because of drainage from the ear (pus).Audiology testing can measure the extent of hearing loss. The goal of treatment is to relieve pain and prevent infection.Occasionally, the health care provider may place a patch over the eardrum while it heals.Surgical repair of the eardrum may be needed, if the eardrum does not heal on its own (tympanoplasty).Warmth to the ear may help relieve discomfort.Call your health care provider if:
You have symptoms of a ruptured or perforated eardrum. You are diagnosed with a ruptured eardrum, and symptoms last longer than 2 months in spite of medical treatment. You are diagnosed with a ruptured eardrum, and you develop persistent fever, general ill feeling, or hearing loss.Do not insert objects into the ear canal, even to clean it.Have ear infections treated promptly.Clinical Assistant Professor, Department of Otolaryngology, State University of New York Health Science Center at Brooklyn, Brooklyn, NY.The medical term for eardrum is tympanic
membrane. The middle ear is connected to the nose by the eustachian tube,
which equalizes pressure in the middle ear.The causes of perforated eardrum are usually from trauma or infection. In medical terms, this is called otitis
media with perforation. Most eardrum perforations heal spontaneously within weeks after
rupture, although some may take up to several months.During the healing process the ear
must be protected from water and trauma. Those eardrum perforations which do not heal on
their own may require surgery. The location of the hole (perforation) in the eardrum also effects the degree of hearing
loss.If the perforated eardrum is due to a sudden traumatic or explosive
event, the loss of hearing can be great and ringing in the ear (tinnitus) may be severe. Before attempting any correction of the perforation, a hearing test
should be performed.Working with a microscope,
your doctor may touch the edges of the eardrum with a chemical to stimulate growth and
then place a thin paper patch on the eardrum.Usually with closure of the tympanic
membrane improvement in hearing is noted.Several applications of a patch (up to three or
four) may be required before the perforation closes completely. There are a variety of surgical techniques, but all basically place
tissue across the perforation allowing healing. The name of this procedure is called tympanoplasty.Surgery is typically quite successful in closing the perforation permanently, and
improving hearing. It is usually done on an outpatient basis.Your doctor will advise you regarding the proper management of a
perforated eardrum.Head and Neck Surgery, Inc.
